Same thing, messing with the fuel and timing maps IS tuning. Basically any modifications to the factory programming is "tuning" but typically the term is reserved for actually manually editing the fuel map not just a "plug in tuner" which often times is just a timing map change and sometimes adds other functionality that didn't exist with the factory tune.
The tune is "flashed" onto the engine management computer (ECM/ECU/whatever the manufacturer decides to call it) by interfacing through the OBD2 diagnostic port.
Plug-in tuners on SOME vehicles also affects the fuel map, but it's a generic tune that doesn't vary from one car to the next.
CUSTOM tune is when someone logs the data for your exact vehicle (plug in a laptop, go for a drive) and edits the base tune to give your engine EXACTLY what it needs.
Plug in tuners also tend to be more destructive to the engine since they are typically just a degree or more of timing and can lead to engine detonation.
Custom tune actually makes the engine run BETTER than it does on the OEM tune.
