You need to say front or rear unless you want to rebuild both; they are different. In any case, just call a dealer for it to be safe.

Our Calipers are very cheap. Like $60ea for the front and that or a little more for the rear. If you are adventurous and wish to rebuild, rebuild kits may not even run $5, the piston between $10 and $15ea. One site that enjoy using is Rockauto.com

It's a heck more than $5 I can tell you that for sure! When you buy the kit, it covers the front or rear pair. I bought one in Canada and it was around $40-50; thanfully got returned as I didn't end up needing it.

How do you know when/if you need to rebuild your brakes?

The caliper kit consists mostly of the rubber pieces; so those rubber pieces can get torn/dry overtime and then you got water/dirt getting where it shouldn't. Probably most people wouldn't know until the pads start to rub but some might inspect the seals and replace them before a real problem develps. Things like the sliding pins are not part of the caliper kit, those are extra items you would need to buy if you need to replace them, as well as the piston.

I was surprised to find the seals on my calipers were in perfect condition after 6 years; but on my other family car they needed to be replaced after 2 or 3 years... so it varies.

I think it's good practice to take apart the brakes at least once a year to re-apply the brake grease on the various parts and lube the piston seal.
