your pads are not properly lubricated. i had the same problem untill my friend who is a mercedes teck brought some grease home. what i found is that the squeak wasnt from the pad and rotor but on the pad and caliper, the pad was moving slightly inside the caliper which made it squeak. if that makes sense. so i pulled the front brakes apart and both rotor and pads were scored. i used a small buffing wheel to take this off and then applied grease on the sides of the pad where it slides into the caliper, than i applied grease on the back side of the pad where the piston applies pressure. and i have put 20k on these same pads and have not heard a single sound.

I went out and did the slam on the breaks thing and it WORKED
My breaks would squeak at low speeds and it was annoying and embarrassing. Now, they make no noise at all and I am proud of myself that I was able to fix yet another problem with my car (I still love my car though).
Anyway, found a backroad, gased it up to 60-70, jumped on the brakes full on until I felt just a bit of abs, and repeated 6 times. Only did one set.
**BEWARE, brake-fade will occur... I have standard 04 brakes, no Brembos. Once the brake fade hits around the third run, the car will take about 5-10feet more to stop which can feel like a helpless feeling, but its ok when you are somewhere where you are not trying to stop before you hit something.
Tested out the brakes against a wall the next day, completely satisfied.
