Clutch, transmission, flywheel, or bearings?

I love my car but the sound and feel of this is killing me. When I first crank it up, clutch is pressed in and I hear a loud grinding/whine. Let the clutch out and push it back in, noise is gone. While clutch is pushed in and I rev my engine above 3 grand it whines again. I can feel it in my pedal as well as my shifter. If I put it into gear and let the clutch out slowly the grind/whine gets really loud and as it catches it goes away. Driving from 1st to 6th it happens. Only when the clutch is pressed in. The throw out bearing was replaced and it still happens. Help!!!
