Cosworth has always been about function and performance over aesthetics. This manifold just happens to look great as an extra benefit! We have seen major gains on just about every forced induction car this has been installed on. On some cars, I'd vouch that the power gain per dollar rivals just about any other single mod out there. We have quite a few case studies and dyno charts available to document some of the specific gains, but one car that comes to mind was spooling earlier and made about 40whp more along the top end of his power curve, and the only change was stock manifold to Cosworth.
For NA gains, it's a toss up. It was designed to be the best flowing manifold for forced induction (which it is, in my experience) but the same characteristics do not always apply to a NA VQ. I have a dyno graph of a Motordyne spacer vs. Cosworth plenum on an otherwise stock car, but the results aren't anything amazing.
