That is neutral in the picture it's way off centered what could cause this?

hmm no clue, i don't think the guide plate being off could cause that but the easiest way to find out is just to take it apart. it is a very easy and quick thing to do.

it went back in place when I start driving it hard. Transmission mount possibly loose?

If it shifts OK, it's likely a broken transmission mount. There is no shift linkage exterior to the transmission, the shifter comes directly out of the transmission housing, so there is no adjustments you can make.
