Consistent Limp Mode Issues (05 Coupe AT)

hey y'all,

Another limp mode issue here. My 05 coupe starts right up, but after two seconds or so it dips down into limp mode. Some days are good, some days are bad, but lately the frequency of this has increased to an every morning circumstance. I will run the three codes, CEL will come on and I'll do an ECU reset and the CEL will go away.

I have since cleaned the MAF sensor and replaced the throttle body, but there are still issues.

What's particularly odd about this is, after I do the idle code (where you hold the pedal down for 20 or so seconds), then crank it up--there's about a second or so where I can rev the engine to over 2500RPM for a bit, and everything is good. almost as if I have tricked the vehicle into getting a bunch of air.

I'm guessing there may be a leak somewhere, or the air filter is a little old. Or maybe I installed the TB wrong (not tight enough or the gasket is off). Will be checking tonight once I get off work.

Thoughts? Ideas? TIA.

Forgot to mention--car is chipped, has a cold air intake, and exhaust mods. Took it down to the dealer months ago and they had no idea due to the vehicle throwing a bunch of different, unrelated codes (their words, not mine).
