A disk offset would be +26 (from pictures shown, this is more acurate)
R disk offset would be +9

rear 19x13, with 6" outer or 150mm
0 disk offset would be +7 (from pictures shown, this is more acurate)

if you read my post again, i merely stated your math doesn't add up:
4" outer with 7" inner does not equal 12"; it equals 11".
