This is the XM unit that plugs into the stock radio. The stock radio will control the unit. All you have to do is plug the unit in in the trunk and put the antenna on the roof/trunk or in the rear window.
I have two antennae that will come with the tuner. One is the original factory pod, which I would use if you are going to put it in the rear window because I think it is way too big to look good on the outside of the car. The other is a regular micro XM antenna that looks much better outside the car. It is magnetic so it will stick on the metal, but I had it stuck to the top center of the rear window with double sided adhesive, which saves paint and looked better.

Mine is paid for by my company, so I can only give you anecdotal answers. I know the regular charge is $12.95 per month, but I have heard that if you press them on it, they will give you a rate of around $80 per year.
If you drive a lot, it's terrific because you don't lose radio stations like normal radio. And, I can only listen to my ipod so manay times...
