Here did a quick research:
If the incident occurred around the city driving:
Submit the claim in writing. Include the time, date and location of the incident, a description of what happened, and a damage estimate.
Mail to:
City Clerk's Office,
City Hall,
100 Queen Street West,
Toronto.
M5H 2N2
Or Fax to: (416) 392-1867
Or you can email: clerk@toronto.ca
Two weeks later, you'll get another letter in the mail acknowledging receipt of your complaint and if it's found valid, you'll then receive yet another form to fill out.

I'm going to mail my $700 bill to replace my windshield to the highway department after a piece of asphalt got lifted from the 427 South on ramp from 401 E and smacked my windshield.
Curious, do you guys think it would be too late to try making a claim if I changed it a month ago?

****ty deal. I had a similar incident, and Ill give you a tip that may help out ALOT.
Have one of your friends call/mail in that pothole size/location today telling the city it is a major hazard.
Then a few days after you call in with a claim, and hopefully get your money.
This summer I was driving and there was a peice of county property in the middle of the road, and due to traffic I had to drive over/ Smash it.
Long story short, the county told me that becuase no one reported the hazard before me, that it wasn't their fault.
Good luck

Thanks to everyone who have provided information regarding this incident.
It boggles my mind that there have been so many incidents with Sevas rims cracking. After seeing all the cracked Sevas rims including mine, I plan on staying far far away from these this manufacturer, unless of course they really did go bankrupt.
As for the pothole I hit, it was brought to the attention of the ministry of transportation well over a week ago. They made a commitment to have the pothole repaired within 5 days, it's now been over a week. Hopefully this is enough to prove negligence on the part of the ministry.
Thanks again, I'll keep you updated on the progress of my claim.

Update
I've contacted the pothole hot line for the city of Toronto and found that this pothole was reported twice, once on March 31st, 2009 and April 2nd, 2009. The latter was my complaint.
I think I have a case on my hands as the ministry gives a 5 day commitment for potholes to be filled. Based on what I was told be the CSR is that it hasn't been fixed as of yet as the ministry plans to repave the entire road and are waiting for the "spring season to come around"
