If the engine went to some insane PRM when the 5th to 2nd downshift occurred (depending on car's speed), then all kinds of things could have been damaged. It could have snapped a cam chain and lunched the whole engine for instance.
It's hard to believe that a piston could kiss a spark plug by stretching the rod enough to do it without something else getting damaged. In order for the piston to kiss the spark plug, there has to be a) lots of rod stretch and/or b) not much clearance between the plug and piston crown. Maybe they guy had some aftermarket spark plugs (4 pronged BS plugs) installed that made the clearance real tight.
