cover in gas tank

cover in gas tank
ok guys so a few weeks ago I went to fill up, and I noticed something. When I removed the gas cap, the metal 'cover' inside was missing (the one that you push the nozzle through)...now I wasn't sure if I was just thinking of my old car or maxima, and at this point I'm still not sure....can someone please confirm for me.
I've noticed a few other problems since then that kind of have me worried like how at gas stations the nozzle stops prematurely before it is fully fueled (it may only put in 7 gallons when the tank is empty and I have to hold the nozzle manually but fuel very slowy) also when I do fill up the tank, the gauge doesn't show its full even tho it is.
I'm out of the factory warranty but when I bought it I purchased an extended bumper to bumper warranty on it so I gotta call up the people and see where I need to take it, but just wanted to get other people's thoughts.
