Please Help! Throttle body help

I tried installing the cooper throttle body heater that came with my motordyne plenum spacers, but while i was attempting to fasten the bolt with a torque wrench i heard a pop. I thought i broke the bold, but after taking it apart again, the bolt seems physically fine. Only the bold that i attempting to tightnen is not holding.

Any idea why this bolt is not holding? What can i do about this? There's no way that i can get away with only 3 bolts fastened correctly?

The bolt hole threads are stripped. You'll need to install a helicoil to restore the threading

Thanks. I tried a different bold that seems to be holding. The torque wrench that i'm using is 3/8 and the socket is resting against the throttle body when i'm tightning..... Will that give me a wrong torque and cause me to over torque? I have a feeling thats what happen the first time because i had the torque wrench at 40 inch when i heard the pop.
