After taking the two halves apart. Mabuchi motor shown.

Here's a photo of the number stamped into the Mabuchi motor.

Here's where I noticed the break in the one piece of plastic. It's possible I broke this when taking it apart, but who knows. Either way it's time for a new actuator.
