Camera is attached to the car via a pole held on by magnets, suctions cups, etc. Some are elaborate, others are pretty simple. Most of the rigs that photogs use on this site are DIY suction cup based. Here is a setup shot of what the rig looks like (w/o the camera attached):

Car is rolled slowly, usually with the engine off, and the shutter is left open anywhere from about 1-30 seconds (longer exposures won't be as sharp) depending on how much background blur is desired and the lighting conditions. Pole is edited out in post processing.
