This form must be a new thing. I got mine last year. It ordered it online and it took ~8 wks, my car is leased and yet I wasn't required to fill out any extra forms.

BTW, on my previous car, I went 1 year before I put on hard plates.
Adversely, If I had a buck for every person I saw driving around with hard (license)plates and the RS (temp) still taped to the inside of their windshield, I'd have a fat wallet.
