3 things it could be, clutch slave, clutch master, and the actual belleville spring in the disc, i have seen all 3 do this with the clutch master cylinder being the most rare only 2 occasions out of the 50+ times i have seen this.
Flushing the fluid to a dot 4 or higher will be a temporary fix.

Before you do anything else, flush of your hydraulic clutch fluid. It is easy and cheap. Here is the link.

I know this seems almost too easy to be true. Trust me, I had my dealer check out the problem and they are clueless. They did a so called "clutch adjustment" that did absolutely nothing.
I was in the process of pricing out a new clutch and Fly Wheel when I came across the above thread. For the hell of it I tried it, and the problem was instantly cured. And that was 40,000 miles ago.
Now I flush my fluid twice a year just to be safe.
