Question Engine behaviour

(This is for a 2008 G35X Sedan)
I'm having a few problems.. I have some ideas what might be wrong but would love some seasoned help from some seasoned vets.
  1. When driving in 1st gear, at around 1.2k RPM, the car will lurch forward and the RPM will 'hiccup'.
  2. The engine will sometimes not idle smoothly. I suspect a vacuum leak [since that what it usually is] but what are some other culprits?
  3. Occasionally, it seems like the exhaust note just disappears in certain powerbands, usually the middle.
Any help would be greatly appreciated, thank you all

Check for vacuum leaks, pull/inspect/replace spark plugs, OBD2 scan for hidden codes (suspecting a cam sensor intermittent failure maybe), clean the MAF. When this is done perform all the resets listed here.
