put in the spacer, but....
Could have happened during initial assembly, or the way the original part was packed or transported. If that's the case, it more than likely passed inspection for fit, form, and function. (hopefully it was inspected..)

It sounds like you were looking at the factory grinding of the lower plenum casting.
If this is what you are looking at, its totally normal. There are six patches of grinding where the casting mold inlets nub need to be ground off.
I see them all the time when working on the MREV2's.
