I would use extended studs for any spacer larger than 3mm.

Since our thread pitch is 12x1.25, then 3mm is over 2 threads, 5mm is 4 threads, not exactly what I would want to lose

the 12x1.25 defines the spacing between threads. 5mm/1.25=4threads. That means you lose 4 turns of the lug. Whats so hard to understand about that?
