My agreement is with "MasterTech", down at the botton of the page:
5 Year New/Used Tire and Wheel Guarantee
REPLACEMENT POLICY: TIRE must be damaged beyond repair or blown out, resulting from a road hazard and have at least 3/32 inch tread depth remaining. Wheel must be damaged so that it is unable to seal with the tire, resulting in air loss.
They paid off on one new OEM tire already, that almost paid for the agreement. Sorry if this applies to you.

EXCEPTIONS AND EXCLUSIONS: Does not apply to: (a) Failure outside the Continental United States and Canada; (b) Destruction due to impact with curb or sidewall damage(does not qualify as road hazard); (c) Tire rot; (d) Rim leaks, Manufacturer defects, vandalism, fire, collision, theft, or misuse occasioned by driving on tire(s) when flat;

did they do a walk around on your car to inspect for preexisting damage before you turned it over to them?
Do you have any recent digitial photos of the wheels?
You are better off making a fuss in the service area than you are by phone.
be prepared to file a grievance with BBB and file a claim in small claims court for the repair. In most states the person named has to appear in court so make sure you get the top guys name at the dealership and service shop.
